Permission Statements Form--CNICCYP 1700/43

Families use this form when they are enrolling and registering their children to document in writing their releases and authorizations for field trips, transportation, topical non-prescription product application, media release, an acknowledgement that they have received the Navy CYP Parent Handbook, and a general hold harmless release. Parents are encouraged to complete the form electronically and then email the completed form to their CYP. A separate form must be completed for each child the family is enrolling in the CYP.
