Follow the steps below to get the forms and documents you need to complete your registration and enrollment.

Download the forms and materials you need.

  • Download these forms onto your computer. (Click on the form and when it opens, save the form/document to your computer. When you fill it out, save it again.)
    • Some forms are fillable. If a form is fillable, complete it and print it with all other documents.
    • If you need help downloading the forms or filling them out, please call your local program. The contact and number is listed in your email.
    • You may also choose to complete your forms at the program office during your Registration Verification visit.

Next, make an appointment with the CDH office.

As instructed by the Provider, you will need to make an appointment with the CDH office for your Registration Verification visit. Please refer to your registration email for detailed instructions and information specifically for you.

Email the completed forms to us.

Once you have completed all forms, save them to your computer and then send all the forms to the email address provided in your enrollment email.

Please email these forms no later than 24 hours before your Registration Verification visit.

When you come to the CYP, bring the following:

  • Proof of eligibility (e.g., military ID card)
  • Your child's immunization records (for children who have not yet started school or who are homeschooled and are not enrolled in any other CYP)
  • Income verification documents (read The Family's Guide to Total Family Income sheet).
  • Notify the CDH Program of an identified special need prior to orientation.
    • Bring necessary documentation, e.g. Individualized Education Program, Emergency Action Plan, to the program.
  • You may return these forms via email in advance.
